Privacy Policy

In March 1996, the new Canadian Standards Association Model Code for the Protection of Personal
Information, CAN/CSA-Q830-96 (the “CSA Code”), was published as a National Standard of Canada.

As a sole proprietor, I am committed to maintaining the accuracy, confidentiality, security and privacy of
my clients personal information. This privacy policy will change and evolve with my business and with
any implemented changes to the code.

I require the following general information from my Reiki clients:

Phone number:
Email address:
Occupation: not required
Allergies:
Medical information if relevant:

Additionally, for my Animal Communications clients:

Breed:
Age:
Medical information if relevant:

The information that is gathered is solely for the purpose of maintaining communication, sharing of
relevant health information is by the choice of the client however, still protected under the privacy act.
Allergy information is required where it pertains to animal dander, use of incense or cleaning supplies.

A signed statement of consent will be required during the first appointment.

Personal information is kept primarily in a digital form, the exception being the signed consent form.
The consent forms will be stored in a locked filing cabinet on premises until the appropriate time to
shred them. The digital information is password protected and will be backed up to disk at regular
intervals and stored off site until requirements allows its destruction. All information provided by the
client is considered confidential and as such will not be shared with any third parties.

Clients may review their records by providing a request in writing and allowing a 2 to 3 week period to
retrieve the requested information. An appointment will be made with the client to hand over copies
and discuss any concerns.
